Moisturizers & Barrier Repair

Best moisturizers for rosacea and perioral dermatitis

This is the one category where doing less is the active intervention.

By Michael W. · Last updated

The short answer

Avene Tolerance Control is the one to reach for if your skin is doing both: fifteen ingredients, no fragrance, no botanicals and no preservative in the declaration, so it adds almost nothing to work out. Vanicream Moisturizing Skin Cream is the cheaper eleven-ingredient version of the same idea. See a clinician about the rash itself - these are what you moisturize with while you do.

Perioral dermatitis - the ring of small bumps and scaling around the mouth, sometimes the nose and eyes - is a different thing from rosacea, and a lot of people have both or get told they have one when they have the other. This page does not attempt to treat it. It answers the narrower question people actually search: what do I moisturize with while this is going on, without making it worse.

The honest answer is that the useful move is subtraction. The strong clinical association with topical steroids, and the commonly reported association with heavy occlusive products around the mouth, both point the same way: fewer things on the skin, for a while. So the picks below are chosen on how little they bring with them, not on what they add.

The screen

Every product on this page had to clear all of the following before it was considered on its merits. This is the exclusion step, and it runs before any recommendation.

  • No added fragrance or parfum anywhere in the ingredient list.
  • No essential oils, including ones declared under a botanical name.
  • No denatured alcohol, witch hazel, menthol, camphor or peppermint.
  • No physical scrub particles.
  • Sold in the US right now, from a brand that publishes its full ingredient list.
  • Every remaining flagged ingredient is named on the product card rather than quietly left out.

Ingredient lists last checked 2026-09-29. Formulas change - if you spot a discrepancy, the published list on the brand's own site wins and we want to know.

What to stop before you start

  • Topical steroid creams, including the hydrocortisone in the bathroom cupboard. Talk to a clinician before stopping if one was prescribed - do not simply swap it for a cream from this page.
  • Inhaled steroids reaching the face, if you use one. Rinsing after a dose is a small change worth making.
  • Heavy balms and oils around the mouth, which is the one place this site's usual occlusion-first advice is contested.
  • Flavoured lip products, especially mint, citrus and cinnamon.
  • Foaming SLS toothpaste, as a two-week experiment. Easy to test, surprisingly often the answer.
  • Actives of every kind - acids, retinoids, vitamin C, benzoyl peroxide. See active ingredients to avoid with rosacea.

That list is longer than the product list below, which is the point. A routine of one cleanser, one cream and one mineral sunscreen is not a compromise here - it is the intervention.

Rosacea or perioral dermatitis?

RosaceaPerioral dermatitis
Where it appearsCheeks, nose, chin, forehead - central faceRing around the mouth, often sparing the skin right at the lip line
What it looks likeDiffuse redness, visible vessels, sometimes papulesClusters of small bumps with fine scaling
FlushingCommon, triggered by heat, alcohol, stressNot a feature
Steroid linkNot typicalStrongly associated, including with withdrawal
Who decidesA clinician. The two overlap and look alikeA clinician. The treatments differ

We are not going to tell you which one you have from a search query. If you have not had it looked at, that is the most valuable step available, because the two conditions are treated differently and one of them gets worse with the treatment people reach for first. The condition literacy pages here - rosacea subtypes explained and sensitive skin vs rosacea - are background, not a diagnosis.

The picks, in order

1. Skin that reacts to almost everything, including preservatives

Avene Tolerance Control Soothing Skin Recovery Cream

A plain cosmetic tube standing upright on a wooden stand

Illustrative photo - not the product.

Fifteen ingredients, no fragrance, and no preservative in the ingredient declaration - it ships in a sterile pump instead. If you have reacted to preserved products, this is the one to try.

Fragrance-free

What is in it

  • Avene thermal spring water
  • Squalane
  • Aquaphilus dolomiae extract filtrate
  • Glycerin

Best for

Skin that reacts to almost everything, including preservatives

Strengths

  • No preservative listed - sterile packaging does that job
  • Fifteen ingredients, no fragrance, no essential oils
  • Squalane and glycerin for comfort without heaviness
  • The shortest list of any cream here after Vanicream

Trade-offs

  • The most expensive per ounce of any pick on this page
  • The sterile pump means you cannot scrape out the last of it

Who should skip it: If cost matters more than ingredient minimalism, the CeraVe or Vanicream creams do the barrier job for a fraction of the price.

2. A barrier rebuild with nothing else in the way

Vanicream Moisturizing Skin Cream

Cosmetic jars and bottles arranged with textured fabric and dried flowers

Illustrative photo - not the product.

Eleven ingredients, petrolatum second on the list, and no fragrance, essential oils or botanicals anywhere. It is an occlusive that seals water in. It is not elegant and it is not trying to be.

Fragrance-free

What is in it

  • Petrolatum
  • Sorbitol
  • Glyceryl Stearate
  • Simethicone

Best for

A barrier rebuild with nothing else in the way

Strengths

  • Eleven ingredients total
  • Petrolatum-led - the most reliable occlusive there is
  • No fragrance, essential oils or botanical extracts
  • Sold in a large tub; cost per ounce is very low

Trade-offs

  • Heavy and slightly waxy - it sits on skin rather than sinking in
  • No soothing or barrier actives beyond the occlusion itself

Who should skip it: If you wear makeup daily or your skin is oily, this will feel like too much - use it at night only, or pick a lighter cream.

Ingredient list read at INCIDecoder - Vanicream Moisturizing Skin Cream.

3. Barrier repair on a budget

CeraVe Moisturizing Cream

A hand holding a plain white cream container

Illustrative photo - not the product.

Three ceramides plus cholesterol and petrolatum, no fragrance, in a tub big enough to use on your body too. This is the cream most dermatology offices hand out, and the formula earns it.

Fragrance-free

What is in it

  • Ceramide NP, AP and EOP
  • Petrolatum
  • Sodium Hyaluronate
  • Cholesterol

Best for

Barrier repair on a budget

Strengths

  • Ceramides NP, AP and EOP with cholesterol - a complete lipid set
  • Petrolatum for occlusion plus hyaluronic acid for water
  • No added fragrance
  • Very low cost per ounce in the large tub

Trade-offs

  • Thick - it needs warming between your palms in winter
  • The tub invites fingers into the product; the pump version is more hygienic

Who should skip it: Oily or acne-prone skin may find the richness too much for daytime; use the lotion version instead.

Ingredient list read at INCIDecoder - CeraVe Moisturizing Cream.

4. A rescue balm for the worst two days of a flare

La Roche-Posay Cicaplast Baume B5

Skincare bottles resting on a soft white towel

Illustrative photo - not the product.

A thick fragrance-free balm with panthenol, madecassoside and shea butter. This is a short-term repair layer for raw, stinging patches, not a daily moisturizer for the whole face.

Fragrance-free

What is in it

  • Panthenol
  • Madecassoside
  • Shea butter
  • Glycerin

Best for

A rescue balm for the worst two days of a flare

Strengths

  • Panthenol and madecassoside - both well studied for irritated skin
  • No added fragrance
  • Thick enough to act as an overnight occlusive layer
  • A small tube lasts a long time in spot use

Trade-offs

  • Heavy and slightly tacky - not a daily all-over cream
  • Contains chlorhexidine digluconate, which a small number of people react to

Who should skip it: If you want one cream for everyday use, this is not it - it is a targeted repair layer.

What we screened out, and why

These are products you will find recommended elsewhere for this exact search. Each one is excluded here for a specific, checkable reason - not because it is a bad product, but because it does not clear the screen above.

  • Heavy occlusive balms over the whole affected area

Occlusion is the right instinct for dry skin and a contested one here - perioral dermatitis is frequently described as responding to a reduction in heavy products around the mouth. We use the lightest cream that keeps skin comfortable instead.

  • Anything containing sodium lauryl sulfate, including toothpaste

Not a moisturizer, but the most commonly missed variable in a rash around the mouth. If your products check out, the toothpaste foaming against your lips twice a day is the next thing to look at.

  • Fragranced lip balms and treatments

They end up on the skin around the mouth all day, and mint, citrus and cinnamon flavourings are among the more reliably irritating things you can put there.

  • Over-the-counter steroid creams used indefinitely

Topical steroids often make a perioral rash look better quickly and are strongly associated with it returning worse when stopped. This is the specific reason this page says see a clinician rather than giving you a treatment plan.

The full list of what we flag, what each ingredient is and where the evidence is genuinely mixed lives on ingredients to avoid with rosacea.

The minimal routine to hold while it settles

  1. 1Morning: rinse with lukewarm water or use a non-foaming cleanser, apply a thin layer of one of the creams above, then mineral sunscreen.
  2. 2Evening: one gentle cleanse, same cream, nothing else.
  3. 3Around the mouth specifically: the lightest layer that stops the skin feeling tight. Not a balm.
  4. 4Nothing new for four weeks. If you add two things and improve, you learn nothing.
  5. 5Keep a one-line daily note. Improvement here is gradual enough that memory is a poor judge of it.

How we chose

Published ingredient lists first, with the shortest lists favoured heavily because that is what this situation calls for. Every pick links to the list we read. No prices are published here - we have no live price feed. Method at how we choose. This page is consumer information about moisturizers, not medical advice, and nothing on it is a treatment for perioral dermatitis.

Questions people ask

What moisturizer is safe for perioral dermatitis?

The one that brings the fewest ingredients with it. Avene Tolerance Control at fifteen ingredients and Vanicream Moisturizing Skin Cream at eleven are the two shortest lists here, both fragrance-free and both free of botanical extracts.

Can I use a thick balm on perioral dermatitis?

We would not. Heavy occlusive products around the mouth are commonly reported as making a perioral rash worse, which is the one place this site's usual occlusion-first advice does not apply. Use the lightest cream that keeps the skin comfortable.

Is perioral dermatitis the same as rosacea?

No, although they overlap and are often confused. Rosacea is central-face redness and flushing; perioral dermatitis is a ring of small bumps and scaling around the mouth. They are treated differently, which is why getting it looked at matters.

Does toothpaste cause perioral dermatitis?

Foaming toothpaste containing sodium lauryl sulfate is a frequently reported aggravating factor, not an established cause. Switching to an SLS-free toothpaste for a couple of weeks is cheap, easy and occasionally decisive.

Should I stop using hydrocortisone cream on it?

Talk to a clinician before you do. Topical steroids are strongly associated with perioral dermatitis and with a rebound flare on withdrawal, so stopping abruptly on your own and hoping a moisturizer covers the gap is not a plan we would recommend.
