Cleansers & Face Wash

Best face wash for rosacea and acne

The combination that gets the worst advice, because the two problems want opposite things.

By Michael W. · Last updated

The short answer

Use a gentle fragrance-free cleanser and treat the breakouts with a leave-on active, not a harsh wash. CeraVe Hydrating Cream-to-Foam is the best compromise for most people. The single most common mistake here is fighting acne with the cleanser.

A minimalist bathroom sink with hand soap and lotion in soft daylight

Rosacea and acne can look similar and want opposite treatment. Acne advice says strip the oil, exfoliate, use benzoyl peroxide. Rosacea advice says protect the barrier and touch it as little as possible. Follow the first set on skin that is doing both, and you get clearer pores on a face that is now permanently red and stinging.

There is also a real diagnostic question underneath. Papulopustular rosacea produces papules and pustules that look a lot like acne but have no comedones - no blackheads and no whiteheads. If you have bumps but no blackheads at all, that is worth raising with a clinician, because the treatments genuinely differ.

The screen

Every product on this page had to clear all of the following before it was considered on its merits. This is the exclusion step, and it runs before any recommendation.

  • No added fragrance or parfum anywhere in the ingredient list.
  • No essential oils, including ones declared under a botanical name.
  • No denatured alcohol, witch hazel, menthol, camphor or peppermint.
  • No physical scrub particles.
  • Sold in the US right now, from a brand that publishes its full ingredient list.
  • Every remaining flagged ingredient is named on the product card rather than quietly left out.

Ingredient lists last checked 2026-09-29. Formulas change - if you spot a discrepancy, the published list on the brand's own site wins and we want to know.

What a wash for both should do

  • Remove sebum and sunscreen properly - a pure cream cleanser sometimes does not, which is why the light-foaming option ranks first here.
  • Leave the barrier intact - no sulfates, no scrub, no stripped feeling afterwards.
  • Carry no fragrance and no essential oils - including tea tree, which the acne aisle is full of.
  • Stay out of the way of your actives - a cleanser with its own acids complicates a routine you are trying to keep debuggable.

The picks, in order

1. Combination skin that wants some foam

CeraVe Hydrating Cream-to-Foam Cleanser

A hand holding a foaming facial cleanser bottle against a pale background

Illustrative photo - not the product.

A cream that foams lightly on contact, with three ceramides and gentle surfactants. The trade against the plainer cleansers here is a much longer ingredient list - it also contains a small amount of salicylic acid.

Fragrance-free

What is in it

  • Ceramide NP, AP and EOP
  • Coco-betaine
  • Sodium cocoyl isethionate
  • Amino acids

Best for

Combination skin that wants some foam

Strengths

  • Three ceramides with mild surfactants rather than sulfates
  • No added fragrance
  • The foam is light, so it cleans without the stripped feeling
  • Removes sunscreen more readily than a pure cream cleanser

Trade-offs

  • A very long ingredient list - more variables than a minimal cleanser
  • Contains a small amount of salicylic acid

Who should skip it: If you are trying to isolate a trigger, this is the wrong cleanser to be using - too many ingredients to rule anything out.

2. The shortest ingredient list on the shelf

Vanicream Gentle Facial Cleanser

A plain white pump bottle casting a soft shadow on pale marble

Illustrative photo - not the product.

Twelve ingredients, no fragrance, no essential oils, no botanical extracts. When skin is reacting and you cannot work out to what, this is the cleanser that removes the most variables at once.

Fragrance-free

What is in it

  • Glycerin
  • Coco-Glucoside
  • Sodium Cocoyl Glycinate

Best for

The shortest ingredient list on the shelf

Strengths

  • Twelve ingredients total - the shortest list of any cleanser here
  • No fragrance, masking fragrance, essential oils or botanical extracts
  • Non-foaming, gentle surfactants (coco-glucoside, sodium cocoyl glycinate)
  • Widely stocked and inexpensive for the size

Trade-offs

  • Does very little beyond cleaning - no ceramides, no soothing actives
  • The lotion texture feels slippery to people used to a foaming wash

Who should skip it: If you wear heavy sunscreen or makeup and want one-step removal, this will not cut through it - double-cleanse or pick a balm.

Ingredient list read at INCIDecoder - Vanicream Gentle Facial Cleanser.

3. Dry, tight, over-washed skin

CeraVe Hydrating Facial Cleanser

Skincare bottles arranged in a woven basket on white bed linen

Illustrative photo - not the product.

A non-foaming cream cleanser carrying three ceramides plus cholesterol - the lipid set your barrier actually uses. It leaves skin feeling comfortable rather than squeaky, which is the point.

Fragrance-free

What is in it

  • Ceramide NP, AP and EOP
  • Sodium Hyaluronate
  • Cholesterol
  • Phytosphingosine

Best for

Dry, tight, over-washed skin

Strengths

  • Three ceramides (NP, AP, EOP) plus cholesterol and phytosphingosine
  • No added fragrance
  • Cleans without stripping - skin does not feel tight afterwards
  • Available almost everywhere, in large sizes

Trade-offs

  • Can leave a slight film on oilier skin
  • Not a makeup remover on its own

Who should skip it: If your skin runs oily through the T-zone and you dislike any residue, a low-foam gel will suit you better.

Ingredient list read at CeraVe - Hydrating Facial Cleanser ingredients.

4. Rinse-free cleansing on a bad flare day

Cetaphil Gentle Skin Cleanser

Pump bottles on a marble bathroom counter in bright daylight

Illustrative photo - not the product.

Ten ingredients, no fragrance, and it can be wiped off without water - genuinely useful when your face is too sore to touch a washcloth. The current formula does contain niacinamide.

Fragrance-free

What is in it

  • Glycerin
  • Panthenol
  • Niacinamide
  • Pantolactone

Best for

Rinse-free cleansing on a bad flare day

Strengths

  • Ten ingredients, no added fragrance, no parabens
  • Can be used without rinsing on days when water makes things worse
  • Panthenol and pantolactone for soothing and moisture
  • Cheap and stocked by every pharmacy

Trade-offs

  • The current formula contains niacinamide - check if that is a trigger for you
  • Leaves a faint residue if you do not rinse

Who should skip it: If niacinamide flushes you, this is now the wrong cleanser - the reformulated version lists it fifth.

What we screened out, and why

These are products you will find recommended elsewhere for this exact search. Each one is excluded here for a specific, checkable reason - not because it is a bad product, but because it does not clear the screen above.

  • Benzoyl peroxide washes

Effective against acne bacteria and genuinely harsh on a compromised barrier. On skin that is doing both things at once, it usually costs more in irritation than it gains in clearance.

  • High-percentage salicylic acid cleansers

A 2% salicylic wash has short contact time and modest benefit, and the surfactant base is usually the aggressive part. If you want salicylic acid, a leave-on at low strength is the better delivery.

  • Scrubs sold as acne cleansers

Abrasive particles do not clear acne and do damage a barrier. This is the worst combination available for this skin.

  • Cleansers with tea tree oil

Tea tree is an essential oil with real antimicrobial activity and real sensitizing potential. On rosacea-prone skin the second usually outweighs the first.

The full list of what we flag, what each ingredient is and where the evidence is genuinely mixed lives on ingredients to avoid with rosacea.

What to treat the breakouts with instead

OptionWhy it fits this combinationThe catch
Azelaic acid 10%Works on papules, pustules and redness at the same time - the only common active with a claim on bothStings for the first week. Usually settles.
NiacinamideOil control plus barrier support, at a gentle strengthA flushing trigger for a minority. Test carefully.
Low-dose salicylic acid, leave-onTargets comedones with controllable frequencyGenuine acne only. Does nothing for rosacea papules.
Prescription topicalsThe most effective route for papulopustular rosaceaRequires a clinician, which is the right call here anyway.

If you are not sure which condition you are treating, that uncertainty is itself the answer: see someone who can look at your face. This site reads ingredient lists - it does not diagnose, and no over-the-counter routine substitutes for prescription treatment when that is what the situation needs.

How we chose

Every cleanser was screened against its published ingredient list first. What remained was ranked on whether it removes sebum and sunscreen adequately without a sulfate base, and on how few extra variables it introduces into a routine that already has an active in it. Full method at how we choose.

Questions people ask

Can you have rosacea and acne at the same time?

Yes, and it is common. It is also common to have papulopustular rosacea mistaken for acne - the distinguishing sign is comedones. Rosacea papules and pustules come without blackheads or whiteheads. If you have bumps and no comedones at all, that is worth raising with a clinician.

Is benzoyl peroxide safe for rosacea?

It is not unsafe, but it is drying and irritating, and on skin that already has a compromised barrier that cost is usually higher than the benefit. Some people with genuine coexisting acne tolerate it at low strength on limited areas. It is a clinician conversation rather than a self-prescribe.

Should I use a salicylic acid face wash?

Probably not. Contact time in a cleanser is under a minute, so the benefit is small, and the surfactant base is usually the harsh part. If salicylic acid helps you, a low-strength leave-on gives you control over frequency and lets you stop instantly.

Why does my acne cleanser make my face redder?

Most acne cleansers are built to strip oil, using strong surfactants and often adding an acid or benzoyl peroxide. On rosacea-prone skin that removes barrier lipids you cannot spare, and the redness is the barrier reacting rather than the acne getting better.

What is the single best active for rosacea with breakouts?

Azelaic acid at 10%. It is the one over-the-counter active with real support for papules, pustules and redness together, which is exactly the overlap this page is about. Introduce it slowly - twice a week over moisturizer to start.
