The short answer
Choose CeraVe Hydrating if your skin is dry and you want ceramides doing barrier work. Choose Cetaphil Gentle Skin if you want the shortest list and the option to cleanse without water on a bad day. Both are fragrance-free; both now contain niacinamide.

These are the two cleansers most likely to be recommended to you, by a dermatologist or by a stranger on the internet, and they are usually presented as interchangeable. They are not. They are built differently and they suit different skin.

The niacinamide detail most comparisons miss
Cetaphil Gentle Skin Cleanser contains niacinamide. That surprises people who remember the old formula, and it matters here because niacinamide is a flushing trigger for a minority of people with rosacea. CeraVe's Hydrating cleanser does not contain it.
This is not a knock on Cetaphil. Niacinamide is well studied for redness and barrier support, and most people tolerate it. What each one is genuinely better at
CeraVe is better if your skin is dry or your barrier is compromised
Three ceramides plus cholesterol and phytosphingosine is close to the lipid set your barrier actually uses. A cleanser is on your face for under a minute, so do not expect a transformation - but it deposits something rather than only removing, and on tight, over-washed skin that shows.
Cetaphil is better if you are troubleshooting, or having a bad week
Ten ingredients means ten suspects instead of twenty-four. And the rinse-free option is genuinely useful: on days when water itself stings, being able to wipe a cleanser off with a soft cloth and stop there is worth more than any active ingredient.

Ten ingredients, no fragrance, and it can be wiped off without water - genuinely useful when your face is too sore to touch a washcloth. The current formula does contain niacinamide.

What about the rest of both ranges?
This page compares the two flagship cleansers, not the brands. Neither brand is uniformly suitable - CeraVe's Renewing SA and Acne lines contain actives that do not belong in a rosacea routine, and Cetaphil's range includes products with quite different formulations. Does Cetaphil contain niacinamide now?
Yes. The current Gentle Skin Cleanser formula lists niacinamide fifth. That is new relative to older formulations, and it matters if niacinamide is one of your triggers.
Are CeraVe and Cetaphil both fragrance-free?
Both flagship cleansers are, with no fragrance or essential oils on their published lists. Neither brand is fragrance-free across its whole range, so check the specific product rather than trusting the brand.
Which is cheaper?
They are close, and both are inexpensive. CeraVe tends to offer a better cost per ounce in the large pump sizes. Neither price is a reason to choose one over the other - the formulation difference matters much more.
Can I use both?
You can, though there is little reason to. If you are trying to work out what your skin responds to, using two cleansers makes that harder rather than easier. Pick one and keep it constant for a fortnight.
